A different focus each week: hitting, pitching, catching, fielding and base running, with HitTrax feedback woven in. Small groups, real individual reps. Instruction by Shea Wardwell — owner of Swingtime, former Red Sox player and Minor League player/coach — plus current and former college and high-school players.

Swingtime Baseball Academy — Clinic Liability Waiver, Disclosure & Parent Agreement

Location: Swingtime Baseball Academy, 6306 Harrison Ave, Cincinnati, OH 45247
Event dates: Saturdays, November 7, 14, 21 and 28, 2026

Liability waiver and release of claims

I, the undersigned parent or legal guardian of the minor named above (“Participant”), understand that participation in the Swingtime Baseball Clinic (“Camp”) at Swingtime Baseball Academy (“Company”) involves inherent risks, including but not limited to physical injuries, accidents, and exposure to contagious diseases (including but not limited to COVID-19).

In consideration for being permitted to participate, I voluntarily agree to assume all risks and release, waive, discharge, and hold harmless Swingtime Baseball Academy, its directors, officers, employees, coaches, volunteers, agents, affiliates, and sponsors (collectively, “Released Parties”) from any and all claims, liabilities, demands, or causes of action arising out of or related to any loss, injury, damage, or death that may be sustained by the Participant or myself in connection with the Camp, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by Ohio law.

Medical authorization

I certify that the Participant is in good physical condition and able to safely participate in the Camp. In the event of injury or illness, I authorize the Company’s staff to provide or seek appropriate medical care, including emergency medical services, and I accept full financial responsibility for any related costs.

I agree that any known allergies, conditions, or medications are disclosed in the medical field of this registration form prior to participation.

Photo release

By attending camp, the Participant’s image and likeness may be captured for social media, video, or marketing materials. By attending camp, you authorize Swingtime to use that content.

Code of conduct and rules agreement

I acknowledge and agree that the Participant must follow all safety instructions and behavioral expectations as outlined by Camp staff. The Company reserves the right to dismiss a participant for inappropriate behavior without refund.

Cancellation and refund policy

I understand that all fees paid are non-refundable unless the event is canceled by the Company. If the Participant cannot attend due to injury or illness, a partial credit may be issued for a future camp at the discretion of the Company.

Governing law

This agreement shall be governed by the laws of the State of Ohio. Any legal action or proceeding arising under this agreement shall be brought exclusively in the state or federal courts located in Hamilton County, Ohio.
